We appreciate your interest as a Subcontractor for DeAngelis Diamond Construction, Inc. Listed below are the documents that are required prior to bidding or performing work on any of our projects. Please fill out and return to us as soon as possible. To the left of this page, you will find some of the documents required (in pdf format).

• Company Profile
  
• Copy of your current Insurance Policy
  
• Certificate of Liability Insurance
  (DDCI must be named as an additional insured once you begin working for DDCI. Payment for work completed will not be issued until DDCI is named as an additional insured.)
  
• Copy of your Certified or Registered Florida license
  (This should include your occupational licenses.)
  
• Contractors Qualification Statement
  
• Form W9 - Request for Taxpayer Identification Number and Certification
  

• Contractual Insurance & Licensing Requirements

• Three letters (on company letterhead) of reference
  Please have the following topics indicated:
  1. General attitude of organization and employees.
  2. Type of work performed.
  3. Your companies ability to maintain schedules.
  4. Quality of work performed.
